    Duties and responsibilities:
    Obtain informed consent from participants visiting the health facility prior to linking patient information in the database.
    Obtain and check the patient files for information required for linkage with the NUHDSS database.

    Identify and link demographic characteristics of patients with the Nairobi Urban Health Demographic Surveillance System (NUHDSS) using Clinic Linkage software in the health facility.

    Update missing personal information required for linkage purposes with the NUHDSS database and document this in the patient file.

    Abstract and enter the patient's health data into the Clinic Link Software.
    Ensure quality data is entered and participate in resolving queries as they arise.
    Ensure the computer is securely stored before leaving the health centre or clinic
    Perform other duties assigned/requested by the supervisor.

    Minimum qualifications

    Diploma or bachelor's degree in health records and information or related field.
    Have proven experience in data collection using electronic data capture tools.
    Fluent in English and Kiswahili (verbal and written).
    Excellent communication, interpersonal and report writing skills.
    Proficiency in Microsoft Office.
